ABOUT HOPE STREET:

Hope Street Youth and Family Services Ltd ("Hope Street") has over 35 years of experience delivering specialist services to young people and their families experiencing, or at risk of, homelessness. 

We are looking for a dedicated Youth Specialist for a permanent full-time position working a regular fortnightly work pattern inclusive of evenings and a weekend, in Brunswick. In this role you will provide specialist case management support to young people and their families, within a holistic client focused approach that includes housing, health, employment, education and community participation. You will proactively engage, motivate and support the young person to identify and work towards achieving their goals. Your relationships with key support services will enhance young people’s connections to those services. You will feel comfortable with maintaining accurate client records compliant with DHHS requirements.

THE BOOST PROGRAM:

The BOOST Program provides case management to young people aged 16-25 who are homeless or at risk of homelessness and who have complex needs, dual diagnosis and challenging behaviour/s. The target group includes young people who are parenting and/or expecting birth.

The BOOST Program provides services clients in crisis, short stay bed (7 night) clients and outreach clients (up to 3 months of service) and operates up to 11pm on weeknights and Saturdays 9-5pm, so that young people can be offered an intensive and responsive service.

ABOUT YOU:

You are a motivated case manager with 2-3 years of experience with relevant tertiary qualifications in Youth Work, Social Work, Psychology or other health/welfare related qualifications. An understanding of the youth homelessness service system and allied services best placed to support and divert young people from homelessness. You have great relationship building skills which enables you to motivate young clients in a crisis setting to achieve meaningful goals. Your friendly and cooperative nature means that you can build relationships quickly with key stakeholders in the community. You pride yourself on your strengths based, solutions focused approach and you value the opportunity to make a real difference in a young person's life.
