Home Care Assistance is an approved provider of Government funded Home Care Packages. We operate on the Mid North and North Coast of NSW. We are currently seeking a Client Care Coordinator for the Mid-North Coast territory (Forster, Taree, Port Macquarie).
The role will involve:
- Managing clients care needs
- Coordinating services and monitoring client budgets
- Employing and scheduling caregivers
- Onboarding prospective clients
- Developing and reviewing care plans
- Communicating and sharing information with established clinical and care management team
This role is home-based starting at 25 hours per week spread over 5 days.
The successful candidate will:
- Have experience as a client care coordinator or be from an allied health background (RN, OT, social worker)
- Be based on the Mid North Coast of NSW (Forster to Port Macquarie)
- Have a valid licence, a car, and a current police records check
- Have excellent communication and interpersonal skills
- Be proactive and diligent in following up client requests in a fast-paced environment
