Role Overview

An exciting opportunity is now available for an experienced Dietitian to join our Aged Care & Chronic Disease team. This role will be offered on a permanent part-time basis, initially located from our Guildford site with a view for relocation closer to the Perth CBD early 2020. If you are a passionate and driven Dietitian who would like to join an organisation focused on their clients, 360 is the place for you!

The Dietitian will work collaboratively with other allied health professionals as part of a close knit team in the delivery of services to those with chronic diseases. This role will also be responsible for assessments, treatment and care of clients referred to 360 Health + Community through grant funded programs, and private services, within this support to individual and group services will be covered.

This role will work in collaboration with other members of the Healthy Living Program and allied health team to improve patient outcomes.

Essential Minimum Selection Criteria

  • Tertiary qualification in Dietetics and DAA membership
  • Demonstrated knowledge of chronic disease management and prevention
  • Clinical experience in case management of patients with cardiovascular disease, type 2 diabetes, chronic renal disease, chronic musculoskeletal conditions and chronic respiratory disease
  • Demonstrated knowledge of primary care and the use of chronic disease Medicare item numbers
  • Experience in providing dietetic consultations to individuals and groups
  • Demonstrated ability to assess client needs within scope of service delivery
  • Ability to work to deadlines and be structured in allocation of time and resources
  • Working knowledge and ability to use IT and clinical software programs for patient management and reporting requirements
  • Ability to contribute to written reports in a concise, professional and appropriate manner
  • Ability to work both independently and collaboratively with other members of the team
  • Ability to maintain accurate records that reflect best practice in service delivery in accordance with 360 clinical governance framework
  • High level of interpersonal and communication skills, and the ability to maintain confidentiality
  • Diplomacy and tact in dealing with all stakeholders
  • National Police Clearance dated within 6 month or willingness to obtain
  • Current valid driver’s license and reliable transport
