About the role

This eclectic, unique and exciting role combines coordination components with hands on support; (health support, social, emotional and transport support), to guarantee a streamlined, cohesive approach to healthy ageing and wellbeing for the Sisters and the Women of the Good Shepherd in Western Australia.

  • Apply a health ageing, well-being approach to supporting the Sisters and the Women (former residents) of the Good Shepherd.
  • Be part of a National team of well-being coordinators around Australia.
  • Utilise wide ranging skills to build strong relationships with internal stakeholders and external care providers
  • Provide hands on support combined with coordination of external providers.
  • Full Time
  • Leederville location

Car travel in and around Perth and very occasionally interstate travel is required. Good Shepherd provides transport support in the form of pool cars and/or km allowances.

There is an on-call support requirement for this role. Re call is generally infrequent.

About you:

You will:

  • Have Qualifications in one or more of Nursing, Allied Health, Community Services, Social work (Essential)
  • Have a background in health services provision to older people; nursing, allied health, social work or other relevant work history
  • Have a minimum of five years aged services experience
  • Have experience working in the community including contact with community services providers; home care package/CHSP providers or other relevant providers (highly desirable).
  • Enjoy working flexibly, with no two days the same.
  • Enjoy a consultative approach with each person at the centre of decision making.
  • Enjoy being part of a small team
  • Have a current drivers licence
  • Have a satisfactory Police Check
  • Have or obtain immediately a current Working with Children’s Check (WWCC)
