About Us

First Step is an addiction, mental health & legal services hub that has been operating in the City of Port Phillip for over 20 years. Our incremental whole-of-person treatment philosophy brings a multi-disciplinary team together with the aim of supporting clients many of whom face multiple challenges that can include mental illness, addiction, homelessness, social isolation and a history of trauma.

We strive to be highly accessible, providing services free of charge and without referral. We operate in a way that allows us to quickly change elements of the treatment team in response to the changing needs of each client.

The First Step family of practitioners extends to GPs, mental health nurses, drug and alcohol workers, psychologists, care coordinators, caseworkers, educators, family therapists and psychiatrists. Uniquely, we also boast a fully integrated pro-bono legal clinic onsite.

The Opportunity

First Step is looking for an exceptional individual to join our growing team. This person will play an integral role in the co-ordination of our highly collaborative, team centred model of care.

Seamless delivery of services to the highly vulnerable cohort of clients at First Step will be the focus of this important new role. They will work in partnership with clients, their caregivers and families and with the clinical & non-clinical team to identify goals, support them with chronic disease self-management including the development of care plans (led by the GP team) and to coordinate services and service providers in order to meet those goals.

At all times the focus of this role will be to deliver high-quality, patient and family centred care.

This is a rare opportunity for the right individual to work alongside and learn from the highly experienced addiction medicine practitioners of First Step that includes: doctors, mental health nurses, psychologists, psychiatrists, caseworkers, family therapists and lawyers among others.

An enrolled nurse with the right experience and attitude would likely be a good fit for this role.

Key Selection Criteria:

  1. Outline your qualifications and experience working in a care co-ordination setting.
  2. Outline any AOD experience or qualifications that you may have.
  3. Compare and contrast your values with those of First Step (as you understand them to be).
  4. Articulate your ability to collaborate enthusiastically and professionally within a multidisciplinary team. Describe your ability to move between different treatment modalities (including addiction medicine).
  5. Outline your recent or ongoing professional development goals.

Terms:

This role is offered on a part-time (3 or 4 days/week negotiable) contract basis for two years until the end of March 2023.
