ABOUT FIGHTING CHANCE:

Fighting Chance designs, builds and scales social enterprises which move the dial for Australians with disability and their family. We work to identify the challenges, hurdles and barriers faced by people with disability in their everyday lives and then design and build sustainable social enterprise businesses to bridge these gaps.

ABOUT THE ROLE:

The Support Coordinator will be a part of a great team who devote themselves to supporting individuals and families to navigate the NDIS system and get the most from their NDIS plans. The Support Coordinator must apply highly attentive interpersonal skills, an in-depth understanding of the NDIS, the community and disability sectors, and expertise in management, planning and administration, to enable each NDIS participant to access the supports and services they need to lead everyday lives and to reach their personal goals.

KEY AREAS OF ACCOUNTABILITY INCLUDE:

  • Strengthening participant's ability to navigate the NDIS and coordinate their supports
  • Liaising with internal and external services to maximise participant outcomes
  • Providing pre and post NDIS planning support
  • Providing support and assistance as appropriate to relevant parties
  • Regularly monitor our participants NDIS plan to ensure services are being delivered within their allocated budget
  • Provide free support to participants and families in line with Fighting Chance’s yes-based culture to support those in the community who are most in need
  • Manage the administration involved in this role, including ensuring that Service Agreements and Schedules of Support are signed for your participants

QUALIFICATIONS AND EXPERIENCE:

  • 1+ years’ experience providing support coordination or case management
  • Mental health experience or qualification (desirable)

SKILLS AND ATTRIBUTES:

  • Excellent interpersonal and communication skills and a strong commitment to customer service
  • Outstanding organisational skills and ability to work in a highly dynamic environment with competing priorities
  • A passion for (and experience with) working with people with disability
  • Understanding of NDIS and the importance of choice and control for participants
  • Ability to work autonomously and self-motivate

REQUIRED TO:

  • Possess the right to work in Australia
  • Possess a current satisfactory National Police Clearance Certificate and working with children check
  • Possess a current NSW drivers licence and registered motor vehicle (km reimbursement allowance)
  • Travel within metropolitan Sydney area
