About the Role

As our Senior Support Coordinator, you will support individuals and families to navigate the NDIS system, and get the most from their NDIS plans.

You will apply your highly attentive interpersonal skills, an in-depth understanding of the NDIS, the mainstream, Community, Mental Health and disability sectors, and expertise in time management, planning and administration, to enable each NDIS participant to access the supports and services they need to lead everyday lives, and to reach their personal goals.

Key duties & Responsibilities

  • Manage a caseload of NDIS participants, to whom you will deliver Support Coordination services
  • Conduct initial assessments of participants, interpreting NDIS plans and health information, as well as personal circumstances, to understand an individual’s support needs
  • Monitor & review of the spending and quality of services and programs being accessed
  • Build and maintain high quality, professional relationships and work closely with family, support networks, and other supports of participants.
  • Act as a subject matter expert on the NDIS & be confident and familiar with key NDIA Legislation and Documentation.
  • Provide leadership on complex cases, and build the capacity of others in the team
  • Provide mentoring, coaching, capacity building support, and clinical supervision to Support Coordinators and other community engagement staff across the Fighting Chance group.

Qualifications & Experience

  • A relevant qualification and/or 6+ years experience in case management/support coordination, or other relevant areas of the disability or community sectors
  • Management or team leader experience
  • Demonstrated knowledge, experience and understanding of the support needs of people with disability, particularly: Those with complex support needs, whose formal support is often a combination of funded NDIS support with other mainstream interfaces
  • Psychosocial Recovery Coaching experience, and demonstrated ability to support complex cases of people whose primary diagnosis is Psychosocial Disability

Required to

  • Possess the right to work in Australia
  • Possess a current unrestricted NSW driver's licence and be able to travel frequently in Sydney, will willingness & ability to use own car
  • Possess or be willing to obtain a NDIS Workers Screening Check
  • Be available & willing to work on occasion, on weekends and evenings
