Independent company looking for a Specialist Support Coordinator to provide case management for individuals with disabilities.

We consist of a small team of passionate coordinators who pride ourselves on quality service provision. We work with people of all ages and who have varying disabilities and needs. We work collaboratively with support providers, allied health professionals and NDIS representatives to ensure our clients are living their best lives.

About the role

Specialist Support Coordination is the coordination of more complex NDIS plans and participant support needs. It involves high level case management with expertise, experience and extensive knowledge of the sector. Specialist Coordinators collaborate with varied community, government, mainstream, education, disability provider and allied health stakeholders.

Specialist Support Coordination is allocated to participant plans by the NDIA. Specialist Support Coordination is different to other reasonable and necessary supports funding under a participant's plan as it is a service with direct contacts to the NDIA and an in-depth knowledge and understanding of the NDIS.

REQUIREMENTS OF THE ROLE:

• Accreditation with Registration body for Social Work (AASW), Psychology (AHPRA) or other related field
• Mental health training, experience and knowledge.
• Experience working effectively with people in the community
• Passion for the disability industry
• Understanding of the NDIS and government funding schemes
• Exceptional communication skills
• Experience in administration and report writing
• Caring, empathic and understanding nature
• Problem solving skills
• Highly organised with great note taking abilities
• Creative, passionate and dedicated
• Dedication and initiative to work independently
