We are currently seeking a proactive and skilled Registered Nurse to join our Virtual Care Team, located at our Business Centre in Coolangatta.

This key role is responsible for providing a range of Telehealth and Clinical services to our clients and their carers/family, using a range of emerging and innovative telecommunication technologies.

This role also provides guidance, support and coordination of clinical interventions to support our clients and assist our staff in the provision of quality care.

About The Role:

  • This is an office based role, and you will be delivering nursing care and advice through videoconferencing, telephone or other technology to support clients and staff
  • Responsible for the clinical oversight of a specific cohort of Feros Care clients (eg in a region, or part of a program), and will work collaboratively with internal and external multidisciplinary teams to ensure clients receive appropriate services to enable them to live independently at home
  • Participate in the provision of holistic client care by addressing and responding to changes in client conditions
  • Liaise with field based staff to ensure appropriate actions are taken in response to a client’s changing condition or care needs
  • Liaise with other health professionals including allied health, medical and hospital staff as required to ensure client needs are understood and appropriately planned for
  • Participate in client care activities including care planning, adjusting client budgets and reviewing information from Community Nurses
  • Promote the use of technology, including monitoring and safety devices to enhance client services and outcomes
  • Provide chronic disease and health education/coaching to clients and their families that considers the cultural, spiritual, intellectual/educational, and psychosocial differences of individual clients, families, and communities
  • Complete documentation ensuring we meet our quality and compliance obligations
  • As you will be delivering care through technology, a basic level of trouble shooting will be required to support the client on first contact, if unsuccessful an escalation process is in place for IT follow up. Training will be provided on this.

About You:

  • Minimum of 2 years’ experience as a Registered Nurse and hold a current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Demonstrated knowledge and experience in contemporary chronic disease management
  • Ability to build quick rapport with client, carer and family
  • Able to use professional judgement during information gathering and decision making
  • Ability to work effectively across a multidisciplinary team - Feros Care Wellbeing Managers, in home Community Clinicians, Care Support Workers and IT colleagues.
  • Strong computer skills and are willing to learn new technologies
  • Excellent administration, communication, and telephone manner

Desirable

  • Recent experience in:
    • Critical Care Experience (Emergency, HDU, ICU, CCU, CICU)
    • Aged Care Experience - Experience and/or an understanding of working in community nursing services
  • Knowledge of My Aged Care process
