Support Coordinators help people with a disability and their families plan, connect to services, overcome barriers and achieve their goals. For many participants and parents, finding their way through the process of planning and connecting with services can be challenging and time consuming. Our focus is on children and young adults and we have strong multi-cultural links. This position will provide Level 2 Coordination of Supports and Level 3 Specialist Support Coordination.
Extended Families has an excellent reputation in quality practice and a supportive team environment. This position will work with complex families and individuals and is ideally suited to an experienced Social Worker or Case Manager. The position is based in our Coburg office, but will involve travel to visit participants in various locations. Whilst our primary cohort is people with intellectual, physical, sensory, neurological disability and Autism, we have an increasing number of people with mental illness so experience in mental health services would be strongly welcomed.

We are looking for people with qualifications in Social Work or equivalent.
The successful applicant will have relevant experience in and understanding of Disability Services, a current Victoria Driver’s license and access to a car, and a current satisfactory Police Check and Working with Children Check.
Ideally the position will be full time, but we are also open to a 4 day a week position.
Support Coordinators work with families, carers and NDIS participants to provide independent support, coordinate all aspects of their NDIS plan and to form links with a range of community, mainstream and funded support services. They help participants and their families to implement their NDIS Plan to achieve goals.
This includes assisting people to:
Orientation, training and ongoing support provided.
You will be based in the Extended Families office in Coburg (and working at home during social isolation). You will be supporting families across metropolitan Melbourne, with an emphasis based where possible on the north west.
Most work is in business hours, although may include occasional after-school or evening visits to connect with relevant family members.
