JOB SUMMARY

The Intake Caseworker is responsible for working directly with new and existing members of the Exodus community to support and identify goals and objectives that meet the individual whilst also facilitating access to any services they need, ensuring guests are holistically supported.This includes being the first point of contact for new and existing guests both face-to-face and via the phone, ensuring a safe and calm environment.

Many of our guests are vulnerable individuals with mental health and substance abuse issues and those who are currently, or at risk of, homelessness. The role will predominantly involve triaging these guests through intake assessments and arranging their support options through the SHW channels and where applicable, referring alternative support agencies. The Intake Caseworker will also hold an additional case load for longer term case plans.

The role will also include some occasional front of house triage and administrative duties – meaning the individual must be versatile, possess a happy and welcoming disposition, good communication skills and be able to work under pressure. At times the position will be extremely busy and will demand a person who can multitask. Most importantly, the Intake Caseworker will always undertake all duties in a caring and supportive manner consistent with Exodus core values to provide an exceptional standard of care to guests.

REPORTING RELATIONSHIPS

The position of Intake Caseworker reports to the Human Services Manager.

DUTIES & ESSENTIAL JOB FUNCTIONS

  • Work with individuals through an intake assessment to deliver outcomes that holistically and flexibly meet the needs of an individual.
  • Referring guests through to casework, counselling, clinical and pastoral support; ensuring mental, physical, emotional and spiritual well-being of guest is appropriately catered.
  • Liaising with, and referring to, internal and external service providers for guests while providing emotional support and relief.  *(Managing and maintaining internal and external stakeholder relationships)
  • Deliver case management activities in accordance with the Exodus Foundation's case management framework and practice approaches.
  • Support staff and volunteers in the delivery of front-line services to the community when required.
  • Ensure all client information and provisions of service are correctly entered into the client database and other reporting mechanisms where required.
  • Participate in team debriefings and meetings
  • Greet guests for triage, answer telephone enquiries and response to referrals.
  • Complete daily reports and checklists before ending each shift.
  • Undertake additional work and training as required.

The successful applicant will have:

  • Two (2) to four (4) years’ experience in similar position
  • Experience and qualifications in responding to challenging behaviour
  • Strong advocacy and referral skills
  • Good data and record management
  • Ability to maintain transparency and “team confidentiality”
  • A thorough knowledge of services and support networks relevant to client group
  • Time Management - Managing one's own time, and the time of others
  • Privacy and Confidentiality
  • Ability to set boundaries when challenged and under pressure
