Our company, Ethical Caring Group works across Ipswich, Brisbane, Toowoomba and the Lockyer Valley. This position will be predominantly working with participants in either the South Brisbane or Lockyer Valley region however you must be willing to travel to other locations for participants outside of these areas. Our Office is based at Rosewood.

We offer a great work-life balance with hours that are flexible however you must have daily availability for phone contact. You will be able to work from home or from our office however you will be required to travel to participant meetings and stakeholder meetings when required.

You will be joining a very friendly and highly passionate team of highly experienced Support Coordinators that support our participants to work toward their individual goals. You will be supported by a manager and an admin assistant.

To be successful for this role you will have:

  • have experience as an NDIS Support Coordinator
  • an exceptional understanding of mental health and other disability groups
  • know how to identify whats supports are needed and where to find the services and programs to meet their needs.
  • have a thorough understanding of strengths-based approach and person-centred care
  • you will be able to think outside the square
  • have a holistic approach to service delivery
  • possess a high level of both verbal and written communication skills
  • experience working with people with a disability and in particular, people with Mental Health needs Intellectual Impairment and Autism Spectrum
  • you will be able to work creatively and resourcefully with participants in how they utilise their support budgets to achieve their goals
  • provide access to mainstream, community, informal and provider options
  • arrange any assessments required to determine the nature and type of funding required (eg assessment to determine the type of complex home modifications required)
  • assist participants to understand and decide the budget for each support type and advise any relevant plan manager of the breakdown of funds
  • experience in providing linking to mainstream or community services (i.e. housing, education, transport, health)
  • develop comprehensive reports to the NDIS

You will also strengthen and enhance participants capacity to coordinate supports, self-direct and manage supports and participate in the community, including providing participants with assistance to:

  • resolve problems or issues that arise
  • help participants and providers to understand their responsibilities under service agreements
  • assisting the participant get ready for their plan review
  • assess whether they achieved their goals and got value for money for their plan
  • identify solutions to problems experienced in implementing the plan
  • consider new goals
  • helping participants decide on what actions to take to achieve goals in relation to exploring housing options and life transition planning.

Essential requirements:

  • open drivers license
  • reliable vehicle that is comprehensively insured
  • current police check
  • blue and yellow card (or ability to acquire)
  • good computer skills with the ability to learn new software programs
  • must be able to work independently and self-direct
  • ability to multi-task
  • must be extremely organised
  • must have your own computer for working at home
  • relevant qualifications such as Cert 4 in Mental Health or similar.
  • in-depth knowledge of the NDIS and experience implementing NDIS plans
  • high level of communication skills

Salary: $35.00 to $45.00 /hour
