About the position

Empower Social Work Services works alongside people, their family and support networks, specialising in acquired brain injury and neurological disorders within the NDIS as well as other funding bodies. You will work within a small team of professionals providing support coordination and specialist services to people with disability. The majority of your time would be spent in the field with clients, liaising with their supports, and engaging with other providers and professionals. You are self-motivated, able to take the initiative and work independently, a great team-worker and a creative problem-solver. Our team is comprised of ethically-minded, empathetic and positive social workers who pride themselves on their willingness to learn, trying new things and getting things done.

Responsibilities:

  • Providing information about funding options, services and community supports available for people living with disability
  • Conducting support needs and risk assessments
  • Enabling clients and their representatives to identify and develop goals, aspirations and goal based plans and outcomes using a person centred and strengths based approach
  • Building client capacity to self-direct services and supports
  • Monitoring and reviewing service plans, goals and budgets in consultation with clients and representatives on a regular basis
  • Maintaining outcome-focused and up-to-date client records
  • Preparing reports for funding bodies
  • Providing advocacy as required, and supporting clients to access funded supports with a strong focus on NDIS application and planning processes
  • Building high quality relationships with clients and their formal and informal supports while maintaining clear professional boundaries
  • Establishing and developing alliances with key health networks, government and community agencies and services

Capabilities:

  • An understanding of, and capacity to undertake, person-centred, goal-directed, strengths-based holistic assessments
  • Excellent written and verbal communication skills, with the ability to communicate sensitively and effectively with clients and others, including individuals from diverse backgrounds
  • Demonstrated mature and professional approach and presentation with the ability to work as part of a team and independently
  • Commitment to excellence in customer service
  • Superior planning, organising and problem solving skills
  • Knowledge of funding bodies and experience in developing networks with relevant agencies
  • Computer literacy skills in MS applications and the internet.

Skills and experience

  • Completion of a degree in social work and possess AASW membership
  • Relevant working and life experience
  • Completion of NDIS training module
  • Current driver’s licence
  • BlueCard – Working with children check
  • You require your own vehicle for this role
  • A computer and phone are provided.
