About the Opportunity

Sunrise Health Service is currently looking for a full-time NDIS Support Coordinator to join their team based in Katherine, NT. You'll be travelling regularly to the Wellbeing Centre in Ngukurr

Travelling regularly to Ngukurr, where the NDIS program is based, this is a hands-on role that will see you providing information and person-centred, culturally-secure support to participants, using a strengths-based approach to help build capacity, connect with services, and navigate the NDIS.

Your key areas of responsibility will include:

• Support design - Conducting needs assessments, working with participants to identify their goals, designing support solutions, and assisting them in understanding their plan, funding, and its purpose. This includes ensuring participants are informed about the range of mainstream and specialist services available to them, helping to develop competencies and resources to enable the participants’ wellbeing and development.
• Connection - Supporting participants to identify support options, creating a support/action plan where practical, and helping them to develop connections with the community, mainstream services, and broader systems of support.
• Reporting - Providing accurate quarterly reports to the NDIA on each participant's progress, as well as providing timely reports to the Program Manager as required. This also includes developing, reviewing and updating the Participants’ Service Agreements or Service Bookings, and making accurate Portal Claims, maintaining detailed notes or records to substantiate these.
• Budgeting - Tracking expenditure for each participant, ensuring services are billed correctly and tracking funds against the participant's plan.
• Collaboration - Establishing strong and effective links with other NDIS and non‐NDIS Service Providers, as well as contributing to the NDIS Program team, and proactively communicating with the Program and Compliance Managers where a concern exists for abuse or neglect, according to mandatory NDIS reporting requirements.

Your dedication will be rewarded with a highly attractive base salary circa $72,560 (highly negotiable depending on your skills and experience) plus super and additional remote area allowances.

If you are proficient in any of the Aboriginal or Torres Strait Islander languages, including Kriol dialetcs used in the Northern Territory, you will also be entitled to an additional bilingual allowance circa $1,450 - $2,900 a year, depending on your level of competency.
