Walgett Aboriginal Medical Service (WAMS) has a fantastic opportunity for a full-time Chronic Disease Manager to join their team in Walgett, NSW.
Reporting to the Chief Executive Officer, the successful applicant will be responsible for managing the Chronic Disease Building and Outreach program while ensuring quality person-centred nursing care through assessment, planning, and implementation in collaboration with multidisciplinary health care teams.
Core duties:
This role leads a number of WAMS' clinical teams and will need to effectively manage community outreach, fly-in specialists, and telehealth services across a number of clinical areas. There is a strong focus on primary healthcare, prevention management and holistic healthcare, so it is important in this role to embrace technology and adaptable methods of healthcare to promote healthy living in the communities.
To be considered for this position you will be an AHPRA Registered Nurse with recognised tertiary qualifications and demonstrable clinical, chronic disease and managerial experience. Experience working in a remote area will also be highly regarded.
Key to your success will be an understanding of the primary health care system and issues relevant to closing the gap in health outcomes for Aboriginal and Torres Strait Islander peoples, including the ability to communicate and appropriately engage with Aboriginal clients.
In addition, the successful candidate will have strong organisational skills and the ability to adapt and effectively manage competing priorities. Leadership and effective communication skills and experience will also be highly valued and will help you hit the ground running in your new role.
Experience using the Communicare system and writing reports will be beneficial, but training will be provided on all systems.
