About the role

Reporting to the CCW Manager and working closely with the Culture & Wellbeing Liaison Officer, the Wellbeing Facilitator is responsible for the assessment of residents wellbeing and lifestyle needs and the development, planning and implementation of innovative programs to meet these needs.

Responsibilities include:

  • Assessment and review of residents needs, covering the whole spectrum of wellbeing, using electronic assessment and care plan tools and documents in ICare computer system
  • Development of group and individual wellbeing programs for delivery by staff or volunteers
  • Documentation of programs, attendance lists and resident care plans to comply with Aged Care Standards and Emmy Monash Policies & Procedures
  • Analysis of the effectiveness of programs to meet resident needs
  • Leadership and mentoring of staff and volunteers

About you

  • Tertiary qualifications in Diversional Therapy, Occupational Therapy, Health/Aged Care Industry or Certificate IV in Leisure & Lifestyle or equivalent;
  • Proven track record and expertise in program development and delivering recreational activities, wellbeing programs, music and art therapy, preferably in an aged care environment;
  • Experience working with older people with dementia and high dependency needs;
  • Experience supervising a team of staff;
  • Highly developed oral and written communication skills and ability to foster effective relationships with staff, residents, family members and external service providers;
  • High level organisational and time management skills;
  • Computer literacy with Aged care software and Microsoft Office suite;
  • Current police clearance certificate and Australian residency or a valid work visa;
  • An understanding of the cultural diversity, festivals, traditions, religious needs and experiences of older Jewish people is a definite advantage
