Due to the ongoing growth and extra demand on support services, we are seeking experienced and newly graduated Support Workers to provide day to day support services ensuring the Elderly and people living with disabilities can live safely and independently within their homes.

We have a number of clients who require a Support Worker who can speak either Indonesian, Malaysian, Cantonese, Mandarin or Hokkien.

Our clients offer a range of services including personal care, medication prompting, transport assistance, respite services, and social programs.

As a caring and compassionate Support Worker, your duties will include:

• Providing support to customers with personal care, living skills, transport, social activities, medication and other support as required
• Providing a person centred approach to help customers achieve their goals and aspirations
• Contributing information for the development of Customer support plans
• Keeping customer records up to date on a daily basis
• Developing and maintain professional relationships and networks with the customers, their families, and key stakeholders
• With support & guidance from the Team Leader, seek out innovative ways to enhance the customer journey

To be successful in this role you must have a caring and compassionate nature, have completed a Cert III in Aged Care, Individual Support or Home and Community Care and have a valid driver’s licence.
