Central and Eastern Sydney PHN is a Not-For-Profit Primary Health Care Organisation covering Sydney Local Health and South Eastern Sydney Local Health District regions.

Our aim is to increase the efficiency and effectiveness of primary health care services for patients particularly those at risk of poor health outcomes; and improve coordination of care.

Purpose of Role

The Health Planner role sits within the Planning and Performance team that has responsibility for assessing the health and health service needs of the population, planning and evaluating commissioned activities, reporting on performance, and data governance within the organisation. The Planning and Performance team is a client focused, collaborative team that works closely with business areas to deliver the best outcomes.

Reporting to the Planning and Performance Manager this role will provide planning and evaluation expertise and high-level advice to management and staff on best practice, supporting the organisation’s vision, goals, values and strategic direction. The Health Planner is responsible for facilitating high quality needs-based assessment and planning, identifying strategies to strengthen the primary health care system, and developing tools to drive and support CESPHN through the commissioning process.

Key Responsibilities

  • Develop and maintain effective working relationships with senior managers, planning and operational staff and other key stakeholders including Local Health Districts (LHDs) and Local Health Networks (LHNs).
  • Lead program specific needs assessments (e.g. health status and service profiles) and facilitate health service planning in accordance with the PHN Needs Assessment Framework, working collaboratively with LHDs, LHNs, local councils and other agencies.
  • Assist with the production of documents including CESPHN’s strategic plans, activity work plans, annual reports and progress reports.
  • Provide high-quality quantitative, qualitative and economic evaluation expertise for PHN programs (including commissioned services) to support evidence-based practice.
  • Undertake quality improvement practices to ensure continuous improvement in all CESPHN processes, activities and deliverables.
  • Remain abreast of relevant policy changes and sector reform and utilise expert knowledge in emerging trends to provide advice.
  • Perform other duties commensurate with skills and experience as directed.

Selection Criteria - Essential

  • Tertiary qualifications in a health-related discipline, population / public health, health planning, and/or research and evaluation.
  • Demonstrated ability to monitor and interpret health related data for the purposes of analysis, planning and evaluation.
  • Sound organisational skills with proven ability to work autonomously as well as effectively as a member of a high performing team.
  • Demonstrated experience in health needs planning, priority setting, monitoring and reporting to support robust and effective primary care service development and delivery.
  • Demonstrated experience in developing and implementing monitoring and evaluation systems to support quality improvement processes.
  • Knowledge and understanding of trends, developments and issues in primary health care, particularly related to planning and evaluation.
  • Excellent interpersonal, engagement and relationship building skills with demonstrated ability to establish confident relationships with both internal and external stakeholders.
  • Comprehensive knowledge of Microsoft Office based computing applications, e.g. Excel, Word, PowerPoint, Power BI.

Selection Criteria - Desirable

  • Experience or knowledge of health funding.
  • Able to manipulate large amounts of information to develop large and complex documents for multiple audiences.
