Purpose of Role

The After Hours program aims to improve access to and increase the efficiency and effectiveness of after hours primary health care services within Central and Eastern Sydney Region.

CESPHN works collaboratively with stakeholders focusing on strengthening pathways, increasing availability and promoting better health outcomes in after hours primary health care. Our focus is person centred and this informs our actions around supporting our regional population, specifically targeting groups who have limited access to health services and are vulnerable.

As part of the After Hours Program you will be responsible for implementing strategies and initiatives that align with the National Health Priorities, CESPHN regional need assessments and the After Hours funding schedule.

The role will work collaboratively with internal and external stakeholders throughout the cycle of commissioning and support the development of regional needs assessments, expansion of evidence-based activity plans, and monitoring and evaluation activities for after-hours services.

The After-Hours Program Officer may also be required to work across the clinical stream portfolio which incorporates mental health, Aboriginal health and Drug and Alcohol activities.

Key Responsibilities

  • Initiate, develop, plan, implement, monitor, and evaluate identified projects that have been reviewed and endorsed by the After Hours Advisory Group
  • Support the development of strategic relationships and partnerships with relevant stakeholders across the CESPHN region, including health professionals, consumers, service users and carers
  • Assist with the development of regional needs assessments and activity plans in conjunction with the Planning, Strategy and Evaluation team and CESPHN Executive
  • In accordance with regional activity plan objectives, support the Manager in commissioning primary health care services to address key service and health care needs
  • Follow CESPHN Commissioning Framework including supporting the co-design process with consumers, service users, carers and youth representatives as required
  • Support Governance of CESPHN commissioned services working with contracted providers to monitor and support activities via collaborative problem solving, contact meetings and data analysis Including the preparation of contract documents and managing the requirements to finalise execution of contracts
  • Establish and manage service agreements, contracts and memorandum of understandings, utilising the Contract Management System as a tool in support of strong governance, monitoring and evaluation
  • Support the collection of data to inform the evaluation processes
  • Implement planned and approved activities undertaken within the disciplines of project management principles and ensure they are delivered on time and within budget
  • Prepare ad hoc reports as necessary, highlighting identified areas for improvement and key trends
  • Contribute to the development and implementation of evaluation of activities working with the Planning, Strategy and Evaluation team, ensuring involvement of relevant stakeholders
  • Participate in internal and external meetings and sector events, delivering formal and informal presentations, chairing and minuting as required
  • Undertake quality improvement practices in accordance with best practice and appropriate accreditation standards
  • Assist with continuing professional development (CPD) events for the health sector
  • Support mental health, Aboriginal health and Drug and Alcohol strategies and activities within the organisation
  • Perform other duties commensurate with skills and experience as directed

Selection Criteria - Essential

  • Tertiary qualifications in an appropriate field and/or relevant experience
  • Experience in delivering programs or services in an efficient, timely manner within budget guidelines
  • Demonstrated ability to establish confident relationships with both internal and external stakeholders
  • Strong organisational skills and the ability to work under pressure and achieve outcomes in complex and challenging environments
  • Proven high level written, verbal and electronic communication skills, with an ability to successfully manage communications across a decentralised organisation
  • Demonstrated ability to manage projects working with internal and external stakeholders
  • Comprehensive knowledge of Microsoft office based computing applications e.g. Excel, Word, Power Point, Share Point
  • Access to a motor vehicle and possession of a current NSW drivers’ licence

Selection Criteria - Desirable

  • An understanding of the primary health care environment and experience in communicating with health care professionals and consumers within CESPHN boundaries
  • Ability and willingness to facilitate educational workshops and develop quality education resources
