About the role:

This is an opportunity to undertake an outreach role within the beautiful Yarra Valley, 4 days per fortnight on week days. The Case Manager/Community Nurse position is an autonomous role, within a multidisciplinary team which provides psychosocial and primary healthcare to Aboriginal clients within the Eastern Health catchment area. This position requires someone with excellent communication skills and creative problem solving abilities. Clinical supervision is provided but the role requires someone with comprehensive clinical knowledge and assessment skills.

• Passion and drive to work in the Aboriginal Community
• Div 1 nurse current registration with AHPRA
• Drivers licence
• High Level of Interpersonal communication and organisational skills
• Ability and experience to work autonomously
• The ability to work as part of a diverse team
