Who we’re looking for

If you're a committed case manager, with experience and/or tertiary qualifications in human services, social work, or other related disciplines, enjoy working closely with other service delivery professionals to support clients experiencing homelessness, you might be exactly the person we're looking for!

We are building a collaborative team of Wellbeing Support Workers with expertise in the homelessness sector to provide outreach to clients, for 18 months, as a part of EACH’s new Homelessness to Home (H2H) program. The Wellbeing Support Worker supports clients with their social and emotional wellbeing, helping clients build resilience and recovery in the areas of mental health, substance use, physical health, and life skills so that they can confidently transition to living in stable accommodation.

You will have the capability to build respectful and collaborative relationships with clients, identifying and resolving and/or reducing barriers to achieving the client’s housing, health, and life goals. You will have experience in a similar role with an outreach focus and a commitment to working as a part of a team with a shared purpose.

What you’ll be doing

As a Wellbeing Support Worker, you will bring a strength-based approach to the role, looking for opportunities to complement and support existing client strengths and capacities. You will work with the client to co-create individual support plans ensuring clients are referred to appropriate supports, engaging other stakeholders, including families, care team members, housing and health services, as needed to improve clients’ physical and mental health and wellbeing.

Through regular interaction with your clients, you will encourage and support clients to access and remain engaged with their current support networks and provide support and connections/referrals to expand these networks where possible.

Who you’ll be working with

Reporting to the Team Lead – H2H, you will work closely with the H2H Team, partner agencies, including Aboriginal Community Controlled Health Services, family violence services, DHHS, and other EACH support services, to achieve positive housing and health outcomes for clients.

The H2H team works with very high levels of flexibility, trust, networking, and collaboration, and members of the team will have strengths in these areas to engage with clients in the community.
