Who we're looking for

As part of our Support Coordination Team at Narre Warren, EACH is looking for a Support Coordinator to provide capacity-building assistance, making a huge difference to the lives of those with developmental delays and disabilities.

What you'll be doing

The Support Coordination role involves working and assisting customers to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan, including mainstream, informal, community, and funded supports. The position also focuses on the provision of high-quality engagement strategies with potential and new customers, building local networks and partnerships, and acting as the key liaison point for all related services chosen by the individual.

The position works with EACH customers to initiate their NDIS plan and develop their skills and confidence to manage their NDIS plan independently. The aim is to assist the customer to build their own capability to take ownership of the Support Coordination function over time, where possible. This role will require the Support Coordinator to work alongside mainstream health services including Mental Health and Disability related services and hospitals. The Support Coordinator will be required to interface with NDIS and mainstream services in a way that demonstrates competency and clarity around navigating NDIS.

The role involves ensuring that customers are provided a service within their funded NDIS Plans, with all services rostered in a timely manner.

Other tasks include:

Face to face and phone contact with NDIS customers, referrals to other organisations, coordinating care team and services, working with the team and team-leads to problem solve and come up with creative solutions, managing customers NDIS budgets, contact with NDIA and attending plan reviews.

Our team comprises of 10 Support Coordinators and a Team Leader available to share information and refer to. You'll have ample opportunities to shape its growth with your experience and ideas. Working with other professionals at Narre Warren within a variety of disciplines, including Occupational Therapists, Physios and Psychologists, our team is dedicated to giving clients the best possible support.
