NDIS Support Coordinator

EACH

Who we are

At EACH, our vision is for a healthy and inclusive community.

Founded on the vision that everyone is entitled to good health, we're one of the nation's leading health and community service providers, offering a broad range of services for over 40 years. Our services include Community Health, Mental Health, NDIS, Counselling, Support for Older Australians and Family Services.

Who we're looking for

Are you passionate about improving the lives of people with disabilities? Do you thrive in teams that encourage you to use your customer-centric approach to enable others to build confidence in their own capabilities? If you're degree qualified in a Health or Human Service related field, with experience in a similar role, enjoy variety in your day, and working collaboratively with other professionals, you might be the next member of our expanding team!

As part of our Support Coordination team based in Southern Melbourne, EACH is looking for an experienced Support Coordinator to join us in this rewarding high-growth area. In addition to your ability to apply customer experience principals to your work, you will have an excellent ability to foster and nurture customer relationships, a desire to help people obtain their personal wellbeing and life goals through an empowerment-based approach, and experience in clinical, educational or community based disability, mental health or early childhood intervention services or related fields.

What you'll be doing

Your day will see you helping clients develop the skills and confidence needed to manage their NDIS plan. Initially your role will be to help clients get started with their NDIS plan; supporting the implementation of all supports in their NDIS plan. You will act as the key liaison point for all related services selected by the individual. Over time, however, you will play an integral role in helping each participant build their own capability to take ownership of the Support Coordination function over time.

Who you'll be working with

Comprised of two Team Leaders, the Support Coordination team is a small, but well-established team in Southern Melbourne. You will primarily work collaboratively with the participant and local service providers. To avoid a conflict of interest, you will work independently of other EACH teams that provide services under NDIS.

What we offer

EACH offers all staff benefits by offering Not for Profit Salary Packaging, a flexible workplace strategy and support through a comprehensive Employee Assistance Program.
