Program Overview

Funded by NSW Department of Communities and Justice the Targeted Early Intervention (TEI) program provides targeted services at the point where they can have the most impact, and deliver the best investment for communities and government.

The TEI Program aims to achieve this through;

• Recognising the significance of culture and identity in safety for Aboriginal children and young people in their family and broader communities.
• Supporting parents to meet the physical and material needs of their children.
• Supporting parents to meet the emotional needs and development of their children through warm and nurturing interactions and encouragement.
• Assist young people to stay connected with their family and make positive life choices as they move into adulthood.

Roles and Responsibilities

The case worker is to actively engage with children, families and communities experiencing vulnerabilities, within the Boggabilla and Toomelah communities, working with them to provide case management services which may involve coordinating service provision across the sector and help youth, families and communities to support and protect their members, through building stronger social connections and support networks.

The Case Worker will:

• Provide initial assessments in a case management setting including case planning to assess the strengths and needs of the young person, including any risks; plan and coordinate a mix of services to meet the young people and family's needs and address risks
• Recognise the impact of trauma on those accessing services, and develop and implement trauma informed policies and practices
• Develop and implement a case plan of individual clients (young persons)
• Provide a case management approach, brief interventions, advocacy, counselling, psycho-education and referral where appropriate
• Work within a harm minimisation framework and using a biopsychosocial approach ensure that Aboriginal children, young people, families and communities have access to timely, effective, accessible and culturally safe support and services
• Deliver a culturally sensitive practice demonstrating best practice, guided by the organisational Policies, Procedures and Model of service and following the policies, guidelines and codes stated in the TEI Program Specifications
• Maintain client contact file notes, case management meetings and treatment plans and monitoring and evaluating the effectiveness of the services being delivered
• Consider risk factors that lead to child abuse, neglect, and domestic and family violence are addressed early; and
• Ensure Aboriginal children, young people, families and communities have access to timely, effective, accessible and culturally safe support and services.
