Your Community Health is seeking an enthusiastic and experienced Peer Navigator to join the Trans and Gender Diverse Health team. This is a part-time position (0.6 FTE - 22.8 hours per week), fixed-term until the end of June 2022.

Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to anyone, through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About the role

Your Community Health is the lead organisation in the Trans and Gender Diverse in Community Health (TGDiCH) Initiative that brings together four of Victoria's key health and wellbeing organisations; Your Community Health, Ballarat Community Health, Thorne Harbour Health and Austin Health to deliver two key components of the Initiative in collaboration with trans, gender diverse and non-binary communities:

  • Multidisciplinary sessional clinics in Preston and Ballarat
  • A state-wide workforce skills and training program

The Peer Navigator will be part of this exciting project, delivering services face to face and via telehealth and will have a key role in supporting trans, gender diverse and non-binary (TGD&NB) people to access and navigate health services, and ensure the involvement of TGD&NB people in co-design of multidisciplinary health service at our PANCH site in Preston.

Your passion and commitment will see you:

  • Developing and implementing effective peer navigation approaches that are evidence-informed and relevant to trans, gender diverse and non-binary (TGD&NB) communities
  • Engaging with the community to promote awareness of the services available to TGD&NB communities
  • Providing a point of contact for TGD&NB people accessing or seeking to access services
  • Providing advice and support as required for initial needs identification for TGD&NB clients accessing services both internally and externally, and provide links to external services as required
  • Providing advice and support for intake, assessment and referral processes with TGD&NB clients

To succeed in this role you will have:

  • Lived experience as a trans, gender diverse or non-binary person
  • Experience (including volunteer-based) working with TGD&NB people in a health or community service environment to support their wellbeing needs
  • Experience providing navigation support, referrals and undertaking administrative processes
  • Sensitivity to the issues faced by TGD&NB people in accessing trans affirmative, non-pathologising health services that support their gender identity and transition goals and an awareness of the impacts on their health and wellbeing created by not being able to secure these essential services
  • Understanding of and commitment to rights-based and person-centred approaches to working with TGD&NB communities, with an emphasis on informed consent and self-determination
