About us

Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to the Trans and Gender Diverse community, through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for the Trans and Gender Diverse individuals of Victoria, providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About the role

Your Community Health is a quality accredited independent community health service with its own Board of Directors. It provides a wide range of allied health, medical, social support and health promotion services. This is a permanent full-time position based in our PANCH health facility based in Preston.

Reporting to Director Community Partnerships the TGDiCH Project Manager role. The Trans Gender and Diverse in Community Health (TGDiCH) Project brings together four of Victoria's key health and wellbeing organisations; Your Community Health, Ballarat Community Health, Thorne Harbour Health and Austin Health in a to deliver two key components of the Initiative in collaboration with trans, gender diverse and non-binary communities

Your passion and commitment will see you:

  • Maintaining and fostering effective relationships with Consortium Partners, TGD community stakeholders and other relevant agencies to ensure successful delivery of the initiative.
  • Providing line management to the peer navigator coordinator based in Your Community Health support as required to the peer navigator at Ballarat Community Health
  • Provide support to the health services at both Preston and Ballarat, to ensure consistent and equitable services are provided to the TGD community regardless of their location
  • Monitoring the implementation of the TGDiCH project plan, including program expenditure, risk management plan and timely completion of milestones.

To succeed in this role you will have:

  • Lived experience as a trans, gender diverse or non-binary person
  • An interest in advocating for the rights of Trans Gender Diver and Non- Binary in healthcare and sharing their lived experience
  • Sensitivity to the issues faced by TGD&NB people in accessing trans affirmative, non-pathologising health services that support their gender identity and transition goals and an awareness of the impacts on their health and wellbeing created by not being able to secure these essential services.
  • Understanding of and commitment to rights-based and person-centred approaches to working with TGD&NB communities, with an emphasis on informed consent and self-determination.
  • A desire to learn how to lead and manage complex projects within a Consortium structure
