Your Community Health is seeking an enthusiastic and experienced Connecting Our Culture, Improving Our Health Worker to join the Aboriginal Health Team, in this full time, fixed-term position to end June 2021.

About us

Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to anyone, through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About the role

Reporting to the Aboriginal Health Coordinator, the Connecting Our Culture, Improving Our Health Worker is responsible for providing comprehensive support Aboriginal and Torres Strait Islander community members to engage with and participate in Aboriginal programs at Your Community Health (YCH). The Connecting Our Culture Worker will also refer Aboriginal and Torres Strait Islander community members to other health services and programs at Your Community Health and other Aboriginal programs and services.

Your passion and commitment will see you:

  • Enhance Aboriginal Programs at Your Community Health
  • Support care coordination

To succeed in this role you will have:

  • Relevant experience and/or qualifications including Aboriginal Health Worker and/or Certificate 3 in Mental Health
  • Experience working with Aboriginal and Torres Strait Islander communities with demonstrated cultural awareness and sensitivity
  • Knowledge of the strengths and challenges in relation to the health and wellbeing of Aboriginal and Torres Strait Islander communities
  • Experience in providing coordinated and comprehensive person-centred care

An Aboriginal and/or Torres Strait Islander person is sought for this role by Your Community Health as a special measure pursuant to Section 12 of the Equal Opportunity Act 2010.
