Your Community Health is seeking an enthusiastic and experienced Carer Support Project Worker to join the Social Support Program team, on a part-time (7 hours per week), fixed-term basis until the end March 2021.

Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to anyone, through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About the role

Your Community Health's "Join" project will engage with and provide a support group for sexually and gender diverse male carers. Reporting to the Social Support Program and Men's Shed Manager, the Carer Support Project Worker will work with group participants to engage with and meet the health and well-being needs of sexually and gender diverse male carers.

Your passion and commitment will see you:

• Ensure participants are actively involved in the development, implementation and review of project
• Work with the participants to increase their capacity to make choices that enhance their health and wellbeing
• Liaise with referral agencies and co-ordinate support services for participants
• Use evidence to support program deliverables

To succeed in this role you will have:

• Community development, health promotion, or other relevant qualifications
• Relevant experience in community engagement and facilitation of active participation with people who are disadvantaged
• Demonstrated and transferable experience working with people who identify as sexually and gender diverse men
• Experience with working with diverse men and communities and volunteers, e.g. CALD groups, people with disabilities, older people and carers
• Excellent time management and communication skills
