Are you experienced in providing culturally safe, holistic trauma informed case work? Want to make a positive impact in reducing homelessness, increasing health and wellbeing and helping people transition back into the community from prison?

The Transition Worker provides holistic, intensive, outreach case work delivering a throughcare model of pre- and post-release support for First Nations people with multiple and compounding needs. The Transition Worker will work alongside people to connect to culture and community and create sustainable pathways away from the criminal justice system. This role operates in Broken Hill with travel to Wilcannia and Menindee.

To join us, you’ll need:

  • Experience and/or relevant qualifications (minimum Cert IV) directly related to the provision of intensive outreach case management support for people with complex and compounding needs such as homelessness, criminal justice system involvement, alcohol and other drug use, mental health issues and experiences of trauma.
  • Demonstrated cultural competency and knowledge of issues relating to Aboriginal & Torres Strait Islander communities
  • Well-developed written and verbal communication as well as computer skills including ability to competently use Microsoft Office including Word, Excel and Outlook
  • Knowledge of current child protection issues and legislation.
  • Driver licence
  • First aid certificate
  • A current NSW Working with Children Check and criminal record check (please note, a criminal record is not a barrier to being considered for a role with CRC)

We request these checks as they are a requirement of the funding contract covering this position. Additionally, anyone entering a correctional centre must undertake a criminal records check to gain authority to enter. Please note, however, that having a criminal record is not a barrier to being considered for a role with CRC.
