We want to hear from people from all backgrounds, including First Nations people and people with lived experience of the criminal justice system and homelessness.
As a Transition Case Worker on our new Reintegration Housing Support Program (RHSP), you’ll be at the heart of a high-impact team. Supported by a Senior Transition Case Worker, you’ll use holistic case management to provide specialist reintegration housing and transitional support for people with complex needs who are leaving NSW Correctional Centres.
Using a throughcare model focusing on the complex transition from incarceration into housing, your role includes:
Your usual work hours will be Monday to Friday, with occasional out-of-hours work, as required.
