The role of Community and Telehealth Registered Nurse

You will connect with clients and staff to achieve positive outcomes

We have an exciting position available for a dynamic and motivated Registered Nurse.

This role forms part of a national telehealth nurse case management pilot program called Patient Pathways being coordinated by the Centre for Community Driven Research.

You will spend two days per week performing telehealth services and the remainder of your time providing clinical and case management advice to CPSN staff.

Key Responsibilities:

  • Oversee and coordinate clinical supervision in accordance within the CPSN clinical governance framework
  • Provide nursing assessments and care planning within a person-centred framework
  • Supervise clinical processes to ensure client needs and plans are met
  • Provide support and advice to a broad range of stakeholders (clients, family members and staff) regarding health care issues
  • Develop and deliver training and assessment programs for including Support Workers
  • Provide clinical advice to CPSN staff related to specific client needs
  • Ensure allied health plans and reports are documented and outcomes monitored

You will also have:

  • Current AHPRA Registration and a Bachelor of Nursing (or equivalent)
  • 5 years' experience in a clinical setting
  • An understanding of the NDIS, disability sector
  • Experience working in paediatric settings
  • Awareness of the challenges faced by individuals living with cerebral palsy/physical disability (desirable)
  • Commitment and drive to deliver quality, human rights-based services in the community
  • High level interpersonal and communication skills
  • High computer literacy with strong database management skills
  • Demonstrated experience establishing, building and maintaining relationships
  • Strong people orientation and a respect for organisational values and culture

This position may require travel to participants homes, therefore you will require a current driver’s licence and your own vehicle. You will be reimbursed for kilometres travelled.
