This role is for a Mental Health Support Worker at Conscious Care Support. As a Mental Health Support Worker, you will support individuals living with a chronic psychosocial disability. You will provide ongoing psychosocial, emotional and practical support to help these individuals identify, work toward and reach their goals.
This is an outreach role. This means that you will independently support individuals in their homes as well as in the community. You will also work collaboratively with the care team of an individual to ensure the best quality of support.
Positions will be working across the outer eastern suburbs of Melbourne.
Conscious Care Support is not structured like other community mental health providers. Whilst we have over 40 employees and service close to 300 individuals each week in the community, everybody at Conscious Care Support provides support work.

At Conscious Care Support, we ensure that all of our mental health support workers are guaranteed consistent, ongoing hours, and an opportunity to autonomously shape and manage a case load that works in accordance with your lifestyle.
We are looking for individuals with at least 3-4 days per week of capacity (weekdays).
