Job Summary & Description

This role is for a Mental Health Support Worker at Conscious Care Support. As a Mental Health Support Worker, you will support individuals living with a chronic psychosocial disability. You will provide ongoing psychosocial, emotional and practical support to help these individuals identify, work toward and reach their goals.

This is an outreach role. This means that you will independently support individuals in their homes as well as in the community. You will also work collaboratively with the care team of an individual to ensure the best quality of support.

Qualifications & Skills

• You will need to own a comprehensively insured car and have a driver license.
• You will need to have a minimum of a Certificate IV in Mental Health.
• You will need to have an ability write case notes and do this articulately and concisely.
• You will need to have a nuanced and detailed understanding about mental health.
• Above everything else, you will need to have a genuine care and compassion to want to help others.
• We are wanting to find individuals who want to stay with us for the long-term and play an instrumental role in our expansion and vision.
