Within our business is the Community Wellbeing Stream, who specialise in the delivery of holistic support to improve individuals health and wellbeing with a range of mental health programs including National Disability Insurance Scheme (NDIS) supports and the National Psychosocial Support (NPS) Transition program.

The NPS Transition program provides support to people with severe mental illness and associated psychosocial functional impairment to transition to the National Disability Insurance Scheme (NDIS) or to access Continuity of Support services.

The Role

We are now looking to hire a Support Worker on a fixed term contract till end June 2020 (with possible extension) to join our Gladstone based team. This role will use recovery based principles to provide one on one support to identify barriers impacting upon mental health and quality of life. This will include support for participants to achieve greater independence, socialisation, education and other personal goals.

Other key duties include:

  • Providing appropriate levels of supports for participants as they test NDIS eligibility to the NDIS and transition to ongoing support either through the NDIS or alternative pathways.
  • Deliver services providing increased opportunities for recovery for people aged 16 years and over whose lives are severely affected by mental illness, by helping them to overcome social isolation and increase their connections to the community.
  • Actively support participants to apply for access into the National Disability Insurance Scheme (NDIS) in line with the full rollout of the scheme.
  • Provision of NDIS services and support for participants that have NDIS packages.
  • Provide direct and personalised assistance through outreach services

About You

The successful applicant will have an understanding of mental health and disability services as well as a knowledge of the National Disability Insurance Scheme (NDIS). You will also hold a minimum Certificate IV Mental Health or Community Services or equivalent and be able to provide a high level of service to both internal and external customers.

You will also come with:

  • Minimum 1 year industry relevant experience and working knowledge of the nature of mental illness and related interventions and techniques for consumers
  • Demonstrated ability to manage high level workloads and meet project deadlines and outcomes
  • Experience maintaining compliance with contractual requirements
  • Competency in computer applications including Microsoft Office

A current and valid driver’s licence is essential, as is a current Blue Card or the willingness to gain one. The successful candidate will also be required to undergo a Federal Police Check.
