Through government funding we are now proud to officially provide Regional Assessment Service, delivering important support assessments to our clients at their homes. As such a fantastic full-time opportunity has now opened up for a talented and hands-on Home Support Assessor to come onboard the team.

In this dynamic and fulfilling role, you'll be kept busy:

• Conducting face-to-face assessments with our clients, building rapport every step of the way
• Developing support plans and referring clients to the appropriate support services under the Commonwealth Home Support Program (CHSP)
• Working towards performance targets and reporting to the Team Leader

To thrive in this role, you'll possess a demonstrated understanding and experience in all of the above. You'll have an excellent grasp of technology to conduct the assessments using our web based databases and software applications. And your energy and exceptional communication skills will see you naturally hitting it off with our clients, developing paramount trust and respect.

A current driver's licence, vehicle and a Working with Vulnerable People (WWVP) card or ability to obtain one is essential.
