Team Leader & NDIS Specialist Coordinator

Communify is a provider of NDIS Support Coordination that has a dynamic and highly experienced team of Support Coordinators. The Team Leader role will support this team and report to the Manager of the Mental Health and Disability Services. This Team Leader role will also ensure that specialist support coordination is delivered for the more complex participant requirements.

Working in a collaborative way the Team Leader will provide supervision, leadership and support to the team members, while working with the participants, carers and the range of community agencies plus clinicians to ensure a high quality of service to the participants.​

To be successful in this role you will need to have:

  • Tertiary qualification in Social Work or relevant field
  • Experience in mental health and/or disability or related field
  • Demonstrated ability to work independently, collaboratively and with initiative
  • Demonstrated team leadership
  • Significant prior knowledge and experience of the NDIS framework and support coordination​

This is a fixed term 12 month full time position with the likelihood of contract extension. ​
