About the role

This position sits within AOD and Homelessness Cluster. The Cluster delivers value-based care through codesign and by developing the capacity and capability for teams to work to the top of their scope of practice and in self-organising ways.

The Community Nurse will use multi-disciplinary, assertive, integrated and flexible care approaches which address immediate health and support needs, coordination and consumer-directed care planning, through to longer-term support to clients with complex presentations, either individually or in a group.

Key accountabilities include:

  • Work closely with health guides and key stakeholders to learn about and understand the community
  • Deliver community health nursing services; assessment, health monitoring and service navigation to residents
  • Provide nasopharyngeal swabbing to collect specimens for Covid 19 testing as required
  • Work in partnership with other HRAR staff if an outbreak response is required
  • Ensure appropriate referral to and utilisation of cohealth and other agency services in the care and support of residents
  • Liaise with other community agencies within the local area to maintain and continue to develop closer partnerships to improve outcomes for high rise residents
  • Work with HRAR community engagement co-ordinator and health guides to support their practice and development
  • Deliver twice-daily briefings for health guides and support their access to health information, training and resources that strengthen their role within local communities

Who are we looking for?

  • Registered Nurse Division 1 with registration with current practicing registration with AHPRA, Minimum 3 years post-graduate experience within a community setting
  • Demonstrates integrity and reliability, and fosters open honest communication, role models a positive outlook even during challenging times
  • Establishes respectful professional relationships that have clear boundaries with residents, staff and partner organisations
  • Demonstrates self-awareness and the ability to self-regulate during difficult situations, a high degree of initiative and the ability to work autonomously as a part of a broader team in a diverse community environment
  • Broad experience working with community members in vulnerable circumstances who have may have experienced issues related to their mental health, family violence or alcohol and other drug use
  • Demonstrated understanding of the social model of health and the importance of community engagement and self-determination
