About the Role

The role of the Specialist Support Coordinator is to provide Support Coordination within a specialist framework, necessitated by specific high level risks in the participant's situation.

This support is time limited and focuses on addressing barriers and reducing complexity in the support environment, while assisting the participant to connect with supports and build capacity and resilience.

By using a community development approach, the Specialist Support Coordinator will build their local knowledge of the available services and will seek to encourage a recovery orientated approach by these services.

The successful applicant will bring solid experience in the community services environment. In particular, they will bring support coordination skills and stakeholder engagement ability and demonstrate an understanding of how best to access, advocate for, and maintain services for participants living with a disability/psychosocial disability who have complex needs. This experience will be complimented by excellent report writing skills and a keen and dedicated commitment to supporting recovery and improving outcomes for people living with disability/psychosocial disability in our community.

You will work with participants, their informal support networks, and collaborate with community partner organisations in order to achieve the participant's goals.

Key Responsibilities

  • Perform specialist support coordination to assist participants in implementing their NDIS plan
  • Coordinate services in participants' NDIS plans including the management of complex multi-disciplinary teams
  • Deliver high quality services in a timely manner including mandatory reporting to the NDIA and meeting CoAbility guidelines
  • Provide advice and consultation with participants, their families and other stakeholders in response to complex and challenging situations
  • Develop action plans following consultation with participants, their families and other stakeholders to resolve challenges arising from complexity
  • Liaise and collaborate with key stakeholders to establish mutual goals of coordinating service delivery for participants
  • Prepare comprehensive reports in conjunction with relevant stakeholders
  • Network and build the profile of CoAbility as a Specialist Support Coordinator service provider

Skills and Experience

Essential

  • A tertiary qualification in one of the below disciplines:
    • Psychology
    • Occupational Therapy
    • Social Work
    • Speech Pathology
    • Developmental education
    • Social and /or health science
  • Membership to with a regulatory body relevant to the tertiary degree (e.g. AHPRA, AASW)
  • Minimum twelve months' experience providing Support Coordination or Case Management with complex clients
  • Knowledge of National Disability Insurance Scheme and relevant legislation including the NDIS Act 2013 and Disability Act 2006
  • Expert communication (both written and oral) skills and able to manage complex relationships
  • Flexible and able to work remotely
  • ABN and appropriate insurances
  • Satisfactory completion of safety screening including a National Police check, International Police check (if required), a clear DWES check, a current Victorian Working with Children Check (Employee), current Victorian Drivers Licence, and the right to work in Australia

Highly Desirable

  • Experience working with people living with a disability/mental health
  • Previous Specialist Support Coordination experience
  • High computer literacy and ability to pick up and master new programs quickly
  • Knowledge of the NDIS Operational Guidelines
  • Ability to work autonomously and as part of a close-knit remote team
