CoAbility is a leading independent NDIS Support Coordination provider in Victoria.

We are a family-run, values-based organisation. Our team of over 70 passionate Support Coordinators and Specialist Support Coordinators are committed to delivering person-centred support and outcomes that make an invaluable impact on the lives of the people we serve.

Importantly, in the current world environment, we have successfully set our team up as a fully remote workforce. Our team has always worked from home, only travelling to attend client meetings.

ABOUT THE ROLE

Due to a substantial increase in our customer base, we are looking for suitably qualified Support Coordinators across Melbourne. The role focuses on addressing challenges, connecting with supports, building capacity and resilience, and achieving the goals outlined in the participant's NDIS plan.

The successful applicant will bring solid experience in the community services environment. In particular, they will bring support coordination skills and stakeholder engagement ability and demonstrate an understanding of how best to access and maintain services for participants living with disabilities. This experience will be complemented by excellent report writing skills and a keen and dedicated commitment to supporting and improving outcomes for the people we serve.

You will work with participants, their informal support networks, and collaborate with community partner organisations in order to achieve the participant's goals.

Participants will already have their NDIS funding plan in place; it will be your job to interpret the plan and work with participants to progress them towards their life goals.

  • A willingness to network and build the profile of CoAbility as a Support Coordination service provider

YOU WILL NEED

ESSENTIAL

  • At least two years experience in the disability/mental health/community services sector
  • Knowledge of National Disability Insurance Scheme and relevant legislation including the NDIS Act 2013 and Disability Act 2006
  • Expert communication skills and ability to manage complex relationships
  • Satisfactory completion of safety screening including a National Police check, International Police check (if required), a clear DWES check, a current Victorian Working with Children Check (Employee),
  • Current Victorian Drivers Licence
  • Right to work in Australia

HIGHLY DESIRABLE

  • A tertiary qualification in a related field
  • Demonstrated experience in case management/supporting people with complex needs, or previous Support Coordination experience
  • High-level computer literacy and the ability to pick up and master new programs quickly
  • Ability to work autonomously and as part of a close-knit remote team
  • Knowledge of the NDIS Operational Guidelines
