Choice Matters is a personalised service connecting NDIS clients to dedicated support professionals. We offer a bespoke service where we match support professionals to people who need them. We involve our clients in selecting who will assist them to meet their goals and aspirations. We offer person centred supports that suit the individual needs of our clients. Choice Matters is committed to fostering a genuine relationship with our clients and our support professionals.

THE ROLE

We are currently looking for professionals to join our team and to provide support to NDIS clients across the Brisbane region. You will be required to:

  • Support to assist OT recommendations and instructions
  • Support the implementation of allied health programs
  • Provide in home support focusing on independent living skills
  • Support personal care and medication administration
  • Support community engagement and areas of interest to the client
  • Keep records of activities (progress notes)
  • Working within the NDIS Code of Conduct

ABOUT YOU

To be successful in this role we are looking for someone who can demonstrate the following attributes:

  • Knowledge and experience of Acquired Brain Injury (ABI) and Mental Health
  • An understanding of NDIS and a commitment to its principles
  • Have an understanding and commitment to a person-centred model of practice
  • Patient and reliable
  • Can listen and know how to have fun and a laugh
  • Can problem solve and use initiative.
  • Great communicators, diligent and well organised
  • Kind, caring, empathetic and have an inclusive attitude.
  • Community connectors
  • Lived experience of ABI (desirable)

ROLE REQUIREMENTS

  • A current Australian Drivers Licence
  • A reliable, registered and insured vehicle (comprehensive insurance is preferred)
  • A smart phone and internet access
  • A Police Criminal Record check from within the last 12 months/yellow card exemption
  • A current Working with Vulnerable People Check (Blue Card)
  • A current First Aid/CPR certificate
  • Evidence that you have completed the NDIS Worker Orientation Module
  • Your Resume and qualifications
