You will be responsible for improving housing outcomes for people exiting prison by providing enhanced intervention and support to ensure access to stable housing to prevent homelessness. Your understanding of issues affecting socially disadvantaged people, as well as awareness of and sensitivity to the needs of people who are in housing crisis and/or have special needs/disabilities, will enable you to excel in this role. This is full time outreach position based at out Morwell office.

You will:

  • Arrange and attend appointments to assess housing and support needs, providing accommodation options for clients as they prepare to exit prison
  • Coordinate housing and support pathways by developing and monitoring individual case plans for clients
  • Assess and refer clients to access appropriate housing pathways based on their needs, including advocating on behalf of clients
  • Determine eligibility and providing access to private rental brokerage opportunities
  • Work closely with tenancy services and support coordination services to ensure the full suite of CHL’s services and external support partners are available to customers
  • Collect data specifically in relation to the Enhanced Housing Pathways program as outlined

What will success look like?

You will demonstrate relevant experience in a similar role, using your knowledge the community and public housing sectors to assess customers and refer them to appropriate services. In addition, you will have great local knowledge of community and support agencies, real estate agents, and resources available to customers. You will also have:

  • A talent for building relationships and working in partnership with a diverse range of stakeholders
  • Excellent communication skills, including the ability to communicate clearly within complex and diverse conversational contexts
  • A client centred approach, including provision of respectful, supportive, and confidential customer service
  • Self-motivated with the ability to work independently as part of a wider team and adapt in a fast-paced environment
  • The ability to achieve work objectives, even in difficult circumstances whilst remaining positive and calm
  • A current Drivers licence and the ability to undertake onsite visits as required
  • A commitment to the right of all people to affordable and sustainable housing.
