About the Role

CBCHS is seeking two dynamic Occupational Therapists to support adults living with disability, chronic illness or age related issues to effectively participate in daily life. This is a community based role, where our OTs work collaboratively with clients, carers, the CBCHS Allied Health multidisciplinary team and external agencies to assist clients in attaining their goals. We are currently accepting applications from both Grade 1 and Grade 2 applicants.

These are permanent positions with 0.8 or 1.0 EFT available for each role.

Some of the key deliverables of this role include:

  • Providing evidence-based OT assessment and intervention to maximise functional independence, safety and quality of life for diverse client groups;
  • Coordinating the provision of equipment and home modifications required for daily living, including completion of applications to funding schemes;
  • Providing care as part of CBCHS Allied Health multidisciplinary team, with a focus on the principles of wellness, reablement and restorative care;
  • Providing services delivered through Commonwealth and State funding, and ‘fee for service’ including NDIS participants;
  • A Grade 2 position would include mentorship and clinical supervision to Grade 1 OT, AHA and students.

About You

Our ideal candidates are qualified, AHPRA registered OT's who:

  • Possess a working knowledge of My Aged Care and NDIS systems with demonstrated experience working and engaging with a range of clients. Prior experience in NDIS highly regarded;
  • Are experienced in community practice, including equipment prescription and home modifications;
  • Possess a strong commitment to delivering high level customer service using a person-centred approach;
  • Are willing to embrace change, to be flexible, adaptable and enjoy new challenges;
  • Are both team players and able to autonomously manage own workloads, completing tasks in timely manner;
  • Committed to continuous improvement in professional practice, knowledge sharing and self-directed learning;
  • Current NDIS Worker Screening Check (or willingness to obtain);
  • Hold registration with Victorian Aids & Equipment Program (Statewide Equipment Program -SWEP) and are eligible for membership with OT Australia.
