We connect carers with a range of state-wide services including support services, counselling, advisory and carer education to help carers navigate the different challenges they may face.

THE ROLE

The Carer Support Advisor provides emotional support, system/sector navigation, information, advice and access/referrals to services for carers.

Your ability to hit the ground running in this fast-paced role is essential. This client-facing, clinical role undertakes a thorough needs assessment that guides the development of a detailed and individualised support plan. As a Carer Support Advisor, you will have a caseload and may broker services, provide emergency service solutions, provide advocacy and emotional support, refer to external and internal services.

Key aspects of the role include:

  • Developing, planning and participating in carer support programs including establishing peer support groups and information and educative programs.
  • Screening, triage, clinical assessment, and care planning; risk and safety assessments, planning and management; other case management tasks including like actioning all aspects of care plans including identifying brokerage needs.
  • Capturing unmet demand (for services) as presented by caring families to support people’s quality of life, physical and psychological wellbeing, social activity and or social connections.
  • Assisting in the coordination of promotion and awareness raising activities across the organisation and to external service providers.

ABOUT YOU

To be successful in this role, you will have:

  • Relevant tertiary qualifications in Social Work, Counselling, Psychology, Community Development or a related discipline.
  • At least one year experience in the community service sector working with carers and families.
  • Demonstrated ability for autonomy, problem solving and decisive clinical action.
  • Demonstrated ability to establish priorities and monitor work-flow within areas of responsibility.
  • Knowledge of the Gateway model and trained in the Carer Star assessment is desirable.
  • Experience with greentree and DC2VUE will be highly regarded.
  • Knowledge of existing service systems and local providers is ideal.
  • Understanding of Child Safety Legislation and a commitment to child safety practice.
