Carers NSW is the peak body in NSW for carers of all ages and circumstances. We are a not-for-profit organisation connecting and empowering carers and improving their quality of life through support, referrals, capacity building and training.
As the Carer Gateway Service Provider (CGSP) in Region 4 of NSW (Central Coast, Hunter New England and North Coast) we are excited to be growing our team and are looking for a team of Mobile Carer Intake & Assessment Outreach Workers to be based remotely within NSW Region 4. We have both part time & full time positions available for those located near Tamworth and throughout the North West area. This is a contract role with the possibility for extension.
Reporting to the Senior Carer Intake Outreach, this role is responsible for connecting with carers to conduct the Carer Support Planning process including intake, registration and needs assessment. The role will also build upon local and relevant services in the community to build awareness and generate referrals to the Carer Gateway. This position will be required to travel with the team to different areas of NSW Region 4.
