As an independent home care specialist, Care Connect have been helping people live independently at home and stay connected with their family, friends and community for over 24 years. We are a not-for-profit organisation approved to provide government funded Home Care Packages and support through the Commonwealth Home Support Program. Through our independent advice and guidance, we tailor a home care plan so clients receive the right services to meet their individual needs and stay living happily at home.

About the Role

We are seeking a kind, enthusiastic and motivated individual with a focus on delivering excellent client outcomes. The role is responsible for the effective management and supervision of a team which may be place-based, virtual, or a combination of both. Includes developing a high functioning, self-motivated team that has synergy across all programs, activities, functions and tasks within the team. The position is responsible for ensuring that a highly effective support service is provided to clients consistent with Care Connect's policies and procedures, and relevant funding contracts.

About You

  • The delivery of exceptional customer and client service
  • Excellent interpersonal communication, networking and negotiation skills
  • Excellent decision making, analytical and organisational skills
  • Experience leading, motivating and developing a high performing team
  • Knowledge of Aged Care Services, Aged Care standards, community resources and service networks for older Australians and their carers
  • Ability to adhere to quality standards and compliance requirements
  • Experience in the Microsoft Office suite and exposure and/or ability to acquire skills for in-house client database systems

Qualifications

  • Tertiary qualifications in a social science, health or welfare discipline, business or a related field
  • In the absence of completed tertiary qualifications, at least 3 years' experience in the delivery of non-complex and/or complex case management or service oriented offerings
  • Understanding of Aged Care sector, aged care standards and legislative framework is desirable

Additional requirements:

  • All appointments are made subject to a satisfactory National Police Check conducted by Care Connect and a valid Working with Children Check provided by the applicant, as requested
  • A statutory declaration is required, confirming residency in a country other than Australia Depending on the information provided a international police check/s may also be required
  • Part of our selection process requires the completion of psychometric assessments
  • A valid driver's licence
