The Social Justice Services HiROADS team, based in Abbotsford, is recruiting a part time Care and Recovery Coordinator to a support a diverse & high risk forensic client population with complex dual diagnosis treatment needs (incl. mental health and alcohol and other drugs) with substance issues and mental illness in their recovery journey.
This position will compliment the therapeutic services provided by SJS clinical staff by providing practical case management and care coordination to clients. Responsibilities will include assessment of needs, assertive referral, individual support (incl. provision of psycho-education), liaising with care teams, waitlist management and building relationships with stakeholders. The role will be based predominantly onsite at Caraniche’s Abbotsford office with occasional travel to other satellite sites as required.
