We are looking for an Evaluation Officer to join the Research Section in the Public Health Division.
The Evaluation Officer will be responsible for the coordination and management of the evaluation of the ITC Care Coordination program at Congress. This position will work collaboratively to co-design and implement a comprehensive evaluation, including community and stakeholder engagement, data collection, data analysis, and all monitoring and reporting activities for the evaluation project.
Integrated Team Care (ITC) Care Coordination involves coordinated, multi-disciplinary care provided by teams of health providers for eligible Aboriginal people with a chronic disease. This evaluation, funded through the PHN ITC Care Coordination Improvement grant, aims to support healthcare providers to implement innovative projects that facilitate service and system improvements, and to improve access for Aboriginal people to culturally appropriate primary health care.
