We’re looking for a professional Community Care Worker with relevant experience who wants to join our 2,000+ team of staff and volunteers to make a difference on a maximum term full time basis, based in Frankston.

THE ROLE

The Community Care Worker will implement The Home and Community Care Program for Younger People (HACC PYP). The funding will be implemented through a Flexible Service Response outreach model that will support clients through outreach activities such as networking, linking in and engaging with other HACC PYP providers in the local area, and designing and evaluating new or innovative service delivery models and approaches for specific groups of HACC-eligible people.

To be considered for the role and be successful in it, we’re looking for an who has the following key skills and experiences:

  • Qualification in Community Services, Bachelor of Social Work or equivalent
  • Demonstrated knowledge of the application of social work theory, practice frameworks in the context of the current community services environment
  • Highly developed interpersonal and communication skills with the ability to work with people from diverse backgrounds
  • Demonstrated ability to work autonomously and as part of a multidisciplinary team
  • Proven problem-solving skills with the ability to make appropriate recommendations
  • Well-developed organisational and time management skills to manage own workload to deliver results
  • Knowledge of and ability to participate in quality improvement activities and strategies
  • Holds a current Victorian driver’s license
  • Understanding of and empathy with the values and ideals of the Brotherhood

Salary will be  $65,872.37 per annum, based on skills and experience, plus superannuation.
