Brisbane North PHN is seeking a Clinical Nurse to join Team Care Coordination. Team Care Coordination is a multidisciplinary team offering coordination support and education to help improve the health literacy and self-management of clients with chronic disease in the Brisbane North catchment area.

About the role

Be part of a committed and caring team that supports and educates people living with chronic health conditions to continue to live well in their own homes in collaboration with General Practice, Queensland Ambulance Service and hospital clinicians.

This is a fantastic opportunity for an individual who enjoys managing their own client caseload while working within a vibrant organisation that is dedicated to improving the primary health care landscape in the region. The successful applicant will be supporting clients from outer northern suburbs of Brisbane and located in the Lutwyche office.

Duties and Responsibilities

  • Provision of phone, telehealth and face to face assessment to deliver comprehensive coordination of health, community and support services for people with chronic health conditions
  • Providing ongoing support to clients and their families during a 12-week episode of care to review the delivery of the health, community and support services for clients enrolled in the team care coordination program
  • Work collaboratively with program referrers and community services to improve the clients care and quality of life. This will include GPs, primary health staff, Queensland health, Queensland Ambulance Service, advocacy services and non-government organisations

  • Registered Nurse with AHPRA registration (or relevant government body)
  • Recent clinical experience with clients with chronic health conditions
  • High level of problem solving and decision making in order to apply appropriate interventions in complex situations
  • Well developed communication skills with the ability to engage effectively with a wide range of stakeholders
  • Ability to be able to work independently and within a high performing team
