About the role

As a Brave Mentor, you will form part of a diverse multidisciplinary team to deliver:

  • Targeted, individual and holistic navigation to participants including development and maintenance of pathway plans.
  • Advocacy services to assist participants and for them to take charge of their circumstances;
  • Strong stakeholder relationships to deliver end to end support to participants.

Skills and Experience

We are seeking applicants who have strong interpersonal and communication skills with a passion for making a difference through providing holistic mentoring and navigation in a primary health care setting. To be you successful, you will need a solution focused attitude, excellent time management and administration skills, and be a motivated self-starter with the ability to work autonomously and as part of an extended team. Previous experience and qualifications in community services, youth work, primary health care and education and/or return to workplace settings would be advantageous. Intrastate travel is required to cover Hobart and surrounding geographic regions.

All successful applicants must hold or be willing to undergo a National Police Check and Working with Children Check prior to commencement. This position is for a fixed term period till January 2021 to assist in the deliverance of a Commonwealth trial project. The extension of the position is subject to further funding.
